Roll a ball from the cradle to the elbow, twist the arm so it's palm up, and roll down to it. Make sure to roll all the way to the elbow before twisting, or the muscles shifting may knock the ball off course.
juggledo: It also looks real nice to do alternating front-back arm rolls, one on each arm. Difficult to get smooth, and even harder to do evenly, but it's a cool effect.
